What we collect
If you fill out the call-back form, we receive:
- Your name
- Your phone number
- The best time to call and the office you prefer
Please don't send medical details through the form. We'll talk about your foot or ankle when we call.
How we use it
- Only to call you back about an appointment.
- We don't sell your information, and we don't use it for advertising.
- The form is delivered to our scheduling inbox through a form service (Formspree), which keeps a copy for a limited time.
Things this website does not do
- No advertising or tracking cookies.
- No patient portal. This website does not hold medical records.
Technical details
Like most websites, the company that hosts this site (Cloudflare) may keep basic technical records, such as your IP address and browser type, to keep the site secure and working. The site's fonts load from Google Fonts. Links to maps and reviews take you to Google, which has its own privacy policy.
Your medical information
Information you share during visits is part of your medical record and is protected under HIPAA. You can ask the office for our Notice of Privacy Practices at any time.
